If you stopped applying patches at some point and those are OS updates to XP itself, it will keep nagging you to put those in. Windows Xp Update Scans from MS, Malwarebytes, and TDSSKiller have come up clean. They don't have to supply anything if you have an old PC.

And as Symantec explained in its blog, spoofing Windows Update is not a trivial matter.
